Year-end Report

As part of the Allocation Process, Agencies must report their performance to pre-established Outcome Measures.  Reporting is required twice a year in September and January.  The September report, “Year-end Outcome Measures Report”, covers the UWGW fiscal year (July 1 to June 30) and the Six-month Outcome Measures Report covers the first half (July 1 to December 31).  The Six-month Outcome Measures Report is submitted with the annual grant application.  The Year-end Outcome Measures Report is submitted separately.

To submit the year-end report, just click on Year-end Report in the Quick Links section above and select “save to file”.  This will save the file to your computer.  After opening the file save it to a new filename using the convention given in the Reporting Instructions on page 2 of the form.
